Step One: Send a referral
You can refer your patient by filling out the form below, by email at referrals@sfstress.com, or faxing it to 415- 799-3736
Please make sure to include:
- Claimant name and DOB
- WC claim number and insurer
- Referring provider name
- Requested service (evaluation or therapy)
- Relevant medical records
Step Two: Scheduling
Our intake team contacts the patient within one business day to start authorization verification and scheduling
